Obtaining Original (Pre-Adoption) Birth Certificate for Adoptees

"Where you came from informs who you are, and every New Yorker deserves access to the same birth records - it's a basic human right," Governor Cuomo said. "For too many years, adoptees have been wrongly denied access to this information and I am proud to sign this legislation into law and correct this inequity once and for all."

Overview

Governor Andrew M. Cuomo signed legislation on November 14, 2019 allowing adoptees for the first time to receive a certified copy of their birth certificate when they turn 18-years-old. This measure (S3419/A5494) helps ensure that all adult New York adoptees will have the same unimpeded right to information about their birth and biological parents.

For Adoptees Born in New York City:

Adoptees born within the five boroughs of NYC should contact the New York City Department of Health and Mental Hygiene, or visit their webpage at for ordering information.

Requirements

Who is eligible to get a copy of a pre-adoption birth certificate?

  • An adoptee who is 18 years of age or older
  • Direct Line Descendants - A Direct Line Descendant is a child, grandchild, or great grandchild, etc. of the adoptee
  • A lawful representative of the adopted person
  • A lawful representative of a deceased adopted person’s Direct Line Descendant

What identification needs to be submitted by the applicant?

乐动体育注册Application must be submitted with copies of either A or B:

  1. One of the following forms of valid photo-ID:
    • Driver's license
    • State-issued, nondriver photo-ID card
    • Passport
    • U.S. Military-issued, photo-ID

OR

  1. Two of the following showing the applicant's name and address:
    • Utility or telephone bill
    • Letter from a government agency dated within the last six months

Please submit a copy of your U.S. passport in addition to the above ID if you are applying from a foreign country that requires a passport for travel.

Location: 800 North Pearl Street, 2nd Floor - Room 200, Menands, NY 12204. (visitor parking, including handicap parking, is available).
Lobby Hours: Monday through Friday, 9:00-3:30 (excluding holidays)
Please Note:乐动体育注册 If you apply via the walk-in window, you will not receive your certificate the same day. Applications received at the walk-in window will be processed in the same manner as mailed in applications.

Third Party Pickup

We do not encourage third party pickups. If you are unable to come to our office yourself, we recommend that you order your certificate by mail or via internet or telephone. More information about third party pickup.

Payment Options:

  • Online / Phone – MasterCard/Visa, Discover, American Express, ACH (Electronic Check)
  • Mail – Personal Check, Postal Money Order, or Certified Check made payable to NYS Department of Health. Payment for orders from foreign countries must be made by a check drawn on a United States bank or by international money order - Do not send cash
  • Walk-Ins - Cash, check or money order. No credit or debit cards
